"Is this covered?" is one of the most common questions we hear — and the honest answer is: it depends on what happens during your visit. Here's a general breakdown.
Typically Covered
- A routine comprehensive eye exam — usually covered at least in part by vision insurance, often with a copay
- Diagnosis and management of a medical eye condition (glaucoma, dry eye disease, diabetic retinopathy, macular degeneration, infections) — typically covered by medical insurance
- Necessary diagnostic testing tied to a medical diagnosis, such as OCT imaging or visual field testing when medically indicated
- Medically necessary contact lens fittings, such as for keratoconus or after certain surgeries
Often Only Partially Covered
- Contact lens fittings for routine correction — vision plans often provide a limited allowance rather than full coverage
- Eyeglass frames and lenses — most vision plans include a set allowance toward frames/lenses, with the patient responsible for anything above that amount
- Premium lens features — such as progressive lenses, anti-reflective coatings, or photochromic lenses, which often carry an additional cost beyond a standard allowance
Typically Not Covered
- Cosmetic procedures like Botox or dermal fillers
- LASIK and other refractive surgery — usually considered elective and excluded from both vision and medical insurance, though many practices (including ours) offer financing options
- Premium intraocular lenses during cataract surgery (beyond the standard lens) — insurance generally covers a standard lens; upgrading to a premium lens (multifocal, toric, or Light Adjustable Lens) typically involves an out-of-pocket difference
- Non-prescription sunglasses
Why It's Worth Checking Ahead
Insurance benefits vary significantly by plan, employer, and even by year. Before your visit, it's worth confirming your specific benefits — including your allowance amounts, copays, and whether prior authorization is needed for any testing.
The Bottom Line
Coverage in eye care isn't all-or-nothing — most patients have some combination of covered and out-of-pocket costs depending on what their visit involves. Our billing team is happy to help you understand your specific benefits before you're seen.
